About the Programme

Welcome to the Care under Pressure e-learning modules. We hope that you find these helpful – now and in the future. You may well wish to revisit all or parts of these modules over the course of your programme, as you encounter different experiences and events.

We recognise that for some, the topics that we will examine may be upsetting at points. If this is the case, we would strongly recommend that you reach out for support. There’s a wide range of confidential sources of support listed within these resources. 

This is an e-learning package designed alongside timetabled sessions to synergistically explore and understand the challenges of foundation programme-based medicine. Importantly, the package then examines how to practically and feasibly enhance foundation doctors’ wellbeing and training experiences. The first module explores the causes and factors involved in promoting or reducing civility and the second module explores active approaches doctors can take to safeguard their wellbeing.


These e-learning modules were created following invaluable research undertaken by a UK based NIHR research programme called ‘Care Under Pressure’. This research used robust, comprehensive methodology to review mental wellbeing support for doctors and provide practical information regarding what works and what doesn’t. The content within these modules is based on their key research findings and themes. These themes centred around how doctor wellbeing isn’t simply a matter of individual resilience and often has very little relevance at all. It is more complex, variable and nuanced. 

The system we practice in is key for both causes and cures of mental wellbeing challenges. However, it is important for us to understand this system, causes and cures for mental wellbeing stressors and be actively involved in shaping change for the better. It is key that those designing systems effectively with meaningful wellbeing support do this in collaboration with you and our wider doctor community. 

Instructors

Dr Josie Cheetham

Josie is a First 5 GP, an honorary clinical lecturer and a clinical academic fellow. She has a Masters in Medical Education and holds special interests in equity, diversity and inclusivity (especially training with specific learning differences), healthcare civility and workforce wellbeing. Josie has held a variety of national leadership positions related to education and training, whilst also designing and delivering a number of study programmes over her 11 years of medical practice. She dedicates her work within these modules to the memory of a past inspirational resident doctor colleague, Dr Almuntassem Kazkaz.

Dr Randika Goonatilleke

Dr Randika Goonatilleke is currently working as the Genito-Urinary Surgical and Administrative Medical Officer at Kings Hospital, Colombo, and a Medical Technology Consultant with a strong focus on health systems innovation and accessibility. With a background in Emergency Medicine, communicable disease prevention, and public health leadership, Randika has worked across both frontline emergency clinical care and community health systems, including leading district-level initiatives in Vavuniya to strengthen community-based healthcare programmes and chronic disease management.

An alumnus of the Healthcare Leadership Academy (HLA) Scholarship Programme, Randika brings a multidisciplinary perspective that bridges clinical practice, technology, and leadership.

Randika’s leadership philosophy centres on empathy, collaboration, and evidence-based decision-making. He believes effective healthcare leadership starts with curiosity and the courage to improve systems, not titles or seniority. His recent HLA project focuses on building an accessible mental health support platform across South Asia, connecting users to therapy, self-help resources, and community support.

Outside of clinical medicine, Randika is passionate about teaching budding new doctors as a medics academy tutor and mentoring emerging healthcare leaders, encouraging innovation, playing strategic board games with friends, and, of course, spending time with his Golden Retriever, Loki.

Dr Amanuel Mulugeta

Dr Amanuel Mulugeta is a physician, medical educator and MPH graduate, with a strong background in clinical medicine, public health, and digital education. He is ECFMG-certified, a credential that verifies his qualifications for medical training and licensure in the United States.

In parallel with his clinical practice, Dr Mulugeta serves as a Clinical Lecturer at Medics.Academy and an Honorary Lecturer at the University of Cumbria (UK). He teaches and designs modules on clinical medicine, technology enhanced learning, artificial intelligence in healthcare, and innovations in medical education.

Dr Mulugeta has mentored both high school students aspiring to enter medicine and current medical students on topics including clinical reasoning, global health, and career development in healthcare. He has led virtual electives and cohort-based teaching, combining case-based methods with personalised guidance. His teaching is known for being interactive, supportive, and forward-looking.

With a passion for integrating modern technology into healthcare education, he is particularly interested in mentoring students exploring medicine, public health, and interdisciplinary pathways such as AI in healthcare.
